-School Consultations: $125

-No Show/Late Cancellation (within 48 hours): $80

-School Accommodation Letter Writing: $35

-FMLA Paperwork Completion: $35

-Diagnostic Confirming Letter and Treatment Recommendations: $35

Sliding Scale - If we do not accept your insurance or if you wish to receive services without the use of insurance, there is a sliding scale that can be discussed.
